Added iterator interface to API, removed cmark_walk.
* Added `iterator.c`, `iterator.h`.
* Removed `cmark_walk`.
* Replaced `cmark_walk` with iterator in HTML renderer.
* Replaced API test for `cmark_walk` with simple iterator test.
